All of us at the First Parish in Lincoln share our heartfelt condolences with you as you prepare for a memorial service. We are honored to support you as you celebrate the life of your loved one. 

Please complete this form to the best of your ability, knowing that we will go over details and questions with you. 

·      Silvia, our church administrator, will follow up with you about building and rental details. 

·      A minister will be in touch with you about planning the service. 

·      A volunteer member of our Memorial Service Committee will be in touch with you about supporting reception planning, if applicable. 

If more than fifity (50) guest are expected, please contact the Lincoln Police Department to hire a detail officer.

In case a Guest Minister or Guest Musicans are chosen, First Parish reserves the right to arrange a meeting  ahead of time. Please submit name, denomination, and contact information.

Please refer to the Catering Policies for any reception planning to offer refreshments.
All cost associated with the reception, including but not limited to catering, paper products, and beverages are the sole responsibility of the hosting party.
The Lincoln Police Department will invoice the hosting party directly following the service.
Please submit payment of all other fees upon acceptance of the Agreement by The First Parish in Lincoln.

The Host understands and agrees to abide by all Policies and Requirements for rentals at First Parish in Lincoln as posted on its website.
Liability statement:
By submitting this Application and Agreement the Host agrees to assume the responsility and legal liability for the above described event and to abide by all policies and requirements for rentals at First Parish in Lincoln as posted on its website upon acceptance of this Application/Agreement by First Parish in Lincoln. 
Additionally the Host agrees to indemnify, defend, and hold harmless The First Parish in Lincoln from any and all  claims for bodily injury or property damage that may arise out of or in connection with this Agreement and use of the premises.
